• When started
• Pain description in words
• How long it lasts each time you experience pain
• Are you taking any medication for pain
• Precipitating factors causing pain
• Relieving factors for pain
• Does pain keep you up at night
• Was there trauma to tooth

Case 2 – A child experiences a fall and cracks a tooth

• When
• Where – examine environment
• How
• Was the child unconscious
• If tooth fell out or part of tooth falls out – which environment did it fall into
• Were there any other injuries to the child
